For Chicago Residents:

Fill out the online forms here to vote by mail in the upcoming March 17th Primary Election or to join the Vote By Mail Permanent Roster.

If you do not want to fill out the online form, you can also download, complete, sign, and submit the paper Vote By Mail Application to the Chicago Board of Elections at:

For Suburban Cook County Residents:

Any registered suburban Cook County voter may request a mail ballot for the upcoming March 17th Primary Election using this online application here. You may also apply for Permanent Vote By Mail status here.

If you do not have one of the online application requirements, or if you prefer to apply on paper, you may download, print and sign a mail ballot application and send it to the Cook County Clerk's office at:

For Lake County Residents:

Lake County residents can apply for a mail in ballot for the upcoming March 17th Primary Election using one of the following option:

1. Online: Log onto LakeVoterPower.info to submit an election application online. For the fastest processing method, complete an electronic application, select “Electronic Request” after log in and follow the prompts through to “Submit Request.” Your signature is not required, and you must complete an attestation.

2. By Mail: Download and complete the Permanent Vote by Mail Application and mail it to:

3. By Phone: Call the Lake County Clerk's Voting by Mail Department at 847-377-VOTE

4. By Email: Email your name, address, and date of birth to votingbymail@lakecountyil.gov.

McHenry County residents can request a vote by mail ballot for the upcoming March 17th Primary Election or Permanent Vote by Mail Status through email, mail, or in person by completing an Application for a Vote by Mail Ballot.
