Any registered suburban Cook County voter may request a mail ballot for the upcoming March 17th Primary Election using this online application here. You may also apply for Permanent Vote By Mail status here.

Lake County residents can apply for a mail in ballot for the upcoming March 17th Primary Election using one of the following option:
Online: Log onto LakeVoterPower.info to submit an election application online. For the fastest processing method, complete an electronic application, select “Electronic Request” after log in and follow the prompts through to “Submit Request.” Your signature is not required, and you must complete an attestation.
By Mail: Download and complete the Permanent Vote by Mail Application and mail it to:
